Project description

RE@HOME addresses the critical and emotional challenge seniors face when transitioning from home care to institutional care. Often, this move results in a loss of personal history, habits, and dignity.

RE@HOME acts as a “Digital Bridge,” connecting families, home care providers, and care institutions. By combining a digital app with professional training, the project ensures that a senior’s personal preferences—from their sleep rituals to their favourite coffee—move with them, ensuring continuity of care and peace of mind for families.

Project objectives

The primary goal is to improve the inclusion, safety, and well-being of elderly people during care transitions.

  • Streamline the flow of information between families and professionals using digital tools.
  • Equip VET educators, professional carers, and families with the skills to provide psychosocial support and person-centred care.
  • Minimize the stress and “institutional shock” for seniors by preserving their personal identity.
  • Create scalable tools that care homes across Europe can easily integrate into their daily workflows.

Project results

The project will deliver four tangible, open-access resources:

  • The RE@HOME App: A free, secure application available in 24 EU languages. It allows families and professionals to record and share essential non-medical data (habits, preferences, routines).
  • The Support Toolkit: A comprehensive training program for VET educators, health professionals, and family caregivers, focusing on empathy and digital skills.
  • The Public Toolkit: A step-by-step implementation guide helping care institutions adopt the RE@HOME methodology and app.
  • The Outreach Platform: A multilingual digital hub hosting all project tools, training materials, and the final Impact Study.
